REQUIRED FORMS:

  • Tuition Agreement

  • Daycare Contract (Policies)

  • NYS Daycare Enrollment Form

  • Medical Statement & Immunizations

  • Child Information

  • Permissions

  • Infant Feeding & Napping Schedules (6w-18m)

OTHER FORMS:

  • Individual Health Care Plan (required for any child with a special need or allergy)

  • Individual Allergy & Anaphylaxis Plan (required for any child with an allergy)

  • Medication Consent form (for emergency medicine only)

  • School Calendar
